Your first visit will involve a consultation to discuss your diagnosis, treatment options and medical history. However, a complex treatment plan or medical history may require additional appointments prior to beginning treatment.
In order to provide as much information to you as possible during your first visit, we need a few things from you.
All patients under the age of 18 must be accompanied by a parent or legal guardian at the first visit. If you are a legal guardian, we will need legal guardianship paperwork. If you are a Power of Attorney, we will need the legal documentation at the first visit.
